First Visit for Physical Therapy in Freeland

Your First Visit

When you contact Functional Advantage Physical Therapy in Freeland you can expect promptness in booking your first appointment and a welcoming atmosphere upon your arrival at our Midland Road clinic (M-47).

You can expect that at your first appointment, your medical history will be taken and a thorough evaluation of your injury or condition conducted. Under normal circumstances, you will also receive your first treatment that day.

Please wear comfortable clothing and bring with you any notes or reports from your doctor. As well, please bring a list of any prescribed or over-the-counter medication you take regularly, your insurance card, and a picture ID.


Functional Advantage accepts Health Plus, Medicare, Blue Cross/ Blue Shield, ASR, Worker Compensation and Auto.

We advise you to contact your insurance company to check on your benefits prior to coming to your first appointment.

We accept cash or checks and will furnish receipts for other insurance providers. Co-pay and co-insurance are due day of service.


Monday to Thursday, 7 a.m. to 6 p.m.

Friday, 7 a.m. to 4 p.m.

Closed Saturday and Sunday


You can make an appointment with Functional Advantage whether or not you have a referral from a physician.

We will help you get the referral process started if you do not have a prescription for physical therapy from your physician.
